Documents Required:

      A. Accepted age proofs (Any one from the following list)-

  1. 10th class mark sheet/certificate

  2. Transfer Certificate from any school for any class with Date of birth mentioned on it

  3. Passport

  4. Affidavit before Notary (For candidates only above 25 years of age)

  5. LIC Bond

  6. Driving License (Karnataka state only)

    B. Permanent address proof (Of Karnataka State Only--Any one from the following list)-

     

  1. Aadhar Card

  2. Voter Id

  3. Passport

  4. LIC Bond

  5. Own house agreement

  6. Own house Electric Bill

  7. BSNL Bill for Telephone, Mobile or Internet

  8. Ration Card

  9. Driving License (Karnataka state only)

    C. Current (Bangalore) Address Proof (If Candidate does not possess any documents from List B as a Current address proof; Any one from the following list to be given along with any one document from list B mandatorily):

     

  10. Letter from HR + Company Id

  11. Affidavit before Notary advocate + Electric Bill

  12. Rental Agreement + Electric Bill

  13. Nationalised bank passbook along with Aadhar card

    D. Photographs:

    2 Passport size photographs During LL test

     

 Note: 

  1. If applicant’s age is 50 years and above, a Medical certificate have to be produced in Form 1A from a registered government practitioner.

  2. For Commercial License (like Cab, LGV, Bus or Truck) minimum educational qualification of 8th standard pass is mandatory and the candidate needs to produce original Marks Card/TC in support of that.

  3. For Commercial License (like Cab, LGV, Bus or Truck) the candidate must have an LMV DL from Karnataka which is at least one year old. Other state DLs must be changed to Karnataka after following a "Change of Address" procedure (Ref S4). The candidate must be of 20 years age

  4. All documents must be produced in original and one photocopy of each must be submitted for both LL and DL test in the RTO. Candidates are requested to carry all original documents and one photocopy of each for both the tests. A signed copy of Form 1 (either supplied from school or downloaded from internet) must be submitted along with the documents

  

S2. Duplicate Driving license

  1. Proof for having lost driving license (Acknowledgement/FIR copy for having lodged complaint with Police)

  2. Affidavit sworn by the applicant before an executive magistrate for having lost the driving license.

  3. Photo copy of lost license (Optional)

  4. 3 recent passport sized photographs.

 

Note: Candidate needs to Visit RTO to have their photo taken

 

S3. Renewal of Driving License

  1. Original existing driving license

  2. 3 recent passport size photographs.

  3. Medical certificate have to be produced in Form 1A from a registered government practitioner.

S4. Change of Address in DL

  1. Address proof: Any one document from List B or List C from sub section S1 above

  2. Original DL

 

Note:

  1. Candidate needs to Visit RTO to have their photo taken

  2. In case of other state DL, Candidate needs to obtain NOC from the RTO from which it was issued

 

S5. International Driving Permit (I.D.P)

  1. 5 recent passport size photograph

  2. Original Valid Driving License from the same RTO

  3. Passport

  4. Visa

  5. Air Ticket

  6. Medical certificate in Form 1A from a registered government practitioner.

 

 Note: 1. Candidate needs to Visit RTO to have their photo taken

           2. The applicant should be citizen of India

           3. The validity of IDP will be of 1 year but will not exceed the validity of the DL

           4.  In case of driving license belonging to other regions/state, the applicant should apply and obtain change of       address in the driving license. In case of license issued by other regions within Karnataka state the time required for change of address is seven working days, and in case of driving license issued by other states the time required is 21 working days. Therefore the applicant keeping the time required in mind should plan well in advance while making an application for IDP.

 

S6. NOC

  1. Form no-28 (3-Copies) with pencil imprint of vehicle chassis number on each of them or 3 imprints on a separate sheet of paper

  2. RC book/card original.

  3. Valid Insurance photocopy.

  4. Valid Emission Test certificate copy.

  5.  Name and address of the person who needs NOC.   

 

Note: The vehicle should not have any loan on it. Otherwise the same must be cleared through the RTO or a consent of the financier must be attached along with the NOC application

 

 

 

S7. RC Book Renew

 1. Sale Certificate in Form No.21

 2. Road worthiness certificate in Form 22.

 3. Identity proof with photographs of applicant.

 4. Vehicle insurance certificate.

 5. Emission Test certificate copy

 5. Proof of address: In case vehicle is imported, then, bill of entry submitted to Customs and in case, vehicle is not a body built vehicle, then Form No.22 Part 2 (A) should also be submitted along with the application. Passport, Electoral Voter I-Card, Electricity Bill with rental agreement, or an Affidavit from the applicant to that effect can be submitted as a proof of address as well as proof of identification.

 

S8. Cancellation of Hypothecation

  1. No Objection Certificate (Issued by Bank).

  2. Form 35 (Both the copies, duly signed by owner).

  3. RC book original.

  4. Valid Emission Test certificate copy.

  5. Valid  Insurance policy - Copy only.

 

S9. Ownership Transfer

  1. Form 29 two copies signed by seller

  2. Form 30 one copy duly signed by seller and purchaser

  3. Original RC

  4. Valid Insurance

  5. Valid Emission certificate

  6. Address proof and id proof of seller and purchaser (ref: list B,C & A respectively of S1)

  7. 3 passport size photographs

  8. For 4 wheeler, CC from originating RTO is mandatory if the transferee resides in another RTO area

Note: the vehicle should be clear of any Hypothecation

 

S10. Change of Address of a Vehicle

  1. Form 28 two copies along with NOC from the RTO where it is registered (Not required for a two wheeler within the state)

  2. Original RC

  3. Valid Insurance

  4. Valid Emission certificate

  5. Current Address proof and Id proof of owner  (ref: list B,C & A respectively of S1)

  6. 3 passport size photographs

Note: the vehicle should be clear of any Hypothecation

S11. Fitness certificate

  1. Application in KMV 20

  2. Original RC

  3. Valid Insurance

  4. Valid Emission certificate

  5. Valid Tax challan

  6. Old Fitness certificate challan (when applicable)
